docs(readme): user-facing DSH compat badge, node-pty-safe install script, paste-to-DSH install prompt

Browse Source 
- Drop peer-floor / CI-baseline jargon everywhere; supported DSH
  versions are stated only via the shields badge
- Install block: add 'pnpm approve-builds --all && pnpm rebuild
  node-pty' unconditionally so the terminal's prebuilt binary is
  always in place (pnpm 11 blocks build scripts by default)
- Add a copy-paste prompt block that lets a DSH session install the
  plugin itself (mirrors the manual script + node-pty step)
- Trim internal process detail from historical changelog bullets
  (v0.14.0 / v0.13.1), keep the user-facing symptoms and gates
